Description of form: In order to protect all students and participants of activities, and to protect East Maiden Baptist Church, all children must have a completed medical release form on file and must be accompanied by a parent/guardian on all activities. The form serves to provide information on current medications, injuries and allergies as well as contact information for parent(s)/guardian(s) in the event of an emergency. Also, it is requested that insurance information be provided as well.
Supervision on trips: Each child is required adequate supervision on all Kid's Korner related activities and/or outings. Please note the space designated on the medical release form for the listing of chaperones for children. We are aware of the conflict arising when parents may not be able to take time away from work. When children wish to participate in an outing sponsored by Kid's Korner Children's Ministry and parents are unable to accompany the child because of work, the approved chaperone list becomes essential.
